Landmark Health was created to transform how healthcare is delivered to the most medically vulnerable members in our community. Our medical group provides home-based medical care to chronically ill patients, many of whom are ill-equipped to navigate our overwhelming healthcare system.

At Landmark, our interdisciplinary teams collaboratively manage our complex patient panels. These teams are led by Physicians, Nurse Practitioners, and Physician Assistants, with supporting care provided by RN Nurse Care Managers, Social Workers, Pharmacists, Behavioral Health, and other employed team members.

The Telephonic Market Outreach Specialist is responsible for increasing patient engagement through telephonic outreach to Landmark members to schedule an in-home clinical visit by a Landmark provider.

This position is full-time (40 hours/week). Employees are required to have flexibility to work any of our 8-hour shift schedules during our normal business hours of Monday- Friday 8:00am-5:00pm or Tuesday-Saturday 10:00am-6:30pm Central Time. It may be necessary, given the business need, to work occasional overtime. Our office is located at 14141 SW FREEWAY SUGAR LAND, TX.

Primary Responsibilities:

• Utilize tried and true telephonic methods of patient outreach and engagement strategies to schedule visits for enrollment into the Landmark Health program.
• Utilize excellent telephonic sales skills to effectively communicate Landmark services and be able to overcome members objections.
• Uses critical thinking skills to create telephonic outreach strategies that are designed to maximize patient awareness and registration into Landmark’s program.
• Leverage reporting and dashboards to prioritize telephonic outreach to members
• In coordination with Local Outreach Team, support telephonic outreach to non-engaged and engaged patients.
• Successfully meet and/or exceed engagement monthly targets
• Reports directly to the Manager, Community Outreach and Engagement

Required Qualifications:

• High School Diploma/GED (or higher)
• Experience in Microsoft Word, Excel and PowerPoint
• Bilingual fluency in English and Spanish
• Ability to work from Monday- Friday 8:00am-5:00pm or Tuesday-Saturday 10:00am-6:30pm Central Time. Including the flexibility to work occasional overtime and weekends as the business need

Preferred Qualifications:

• Bachelor’s Degree (or higher)
• 2+ years of related experience in a sales, direct marketing or customer service capacity
• Experience in healthcare

• Reside within a commutable distance of Houston, TX
• Required to have a dedicated work area established that is separated from other living areas and provides information privacy
• Ability to keep all company sensitive documents secure (if applicable)
