Job Summary

This position is responsible for leading the execution and performance monitoring of social health strategies and l priorities for the organization. This involves working collaboratively with CCO leaders, delivery system partners, and social service agencies to develop and implement social health levers in order to address the social needs of our members. Time is focused on business group oversight, as well as enterprise-wide engagement. Primary duties include operational planning and oversight, as well as resource, relationship, and people management. This position provides input into strategic plans for the broader organization.

Essential Responsibilities

Technical/Operational Leadership

• Provides operational leadership and oversight of the organization’s social-based programs serving CareOregon members.
• Drives the implementation of the Social Health Strategy:
  • Develop, implement, and monitor the Health Related Social Needs Benefit
  • Optimize Health Related Services and coordinate with operational partners
  • Utilize data to drive strategy and quality improvement
  • Optimize closed-loop social needs referral technology
  • Support delivery system in implementing and sustaining social needs payment transformation models
  • Establish meaningful community partnerships and direct technical assistance delivery include contract development and member eligibility protocols.
  • Direct meaningful evaluation for social health programs
  • Develop and support a high quality and diverse workforce to meet members’ social health needs
• Designs and implements team performance monitoring system to ensure program effectiveness, fidelity to best practices, and regulatory compliance.
• Utilizes available resources to accomplish the goals set out in the strategy and work closely with regional leadership to deploy interventions and strategies in regionally meaningful ways.
• Leads the team effectively through change; regularly and proactively communicate with leaders and staff regarding impending external and internal change. Include front line staff whenever possible in designing solutions to effectively navigate change.
• Collaborates with other CareOregon departmental leaders to develop cross-departmental social health strategies that leverage multiple disciplines and promote efficiency for the organization.
• Collaborates with delivery system leaders within all CCO regions to build a strong foundation of social health program support for CareOregon’s statewide primary, secondary and tertiary delivery systems.

Strategic/Operational Planning

• Participates in the development of vision, goals, and strategic plans for Social Health.
• Provides input into the strategic plans of the organization.
• Develops annual team goals that align with organizational strategic goals in collaboration with the Vice President, Population Health.
• Develops short and long-term plans and policies; oversees the development and execution of standard operating procedures.
• Maintains a business unit view while establishing department priorities, being cognizant of broader business unit and organizational impacts.

Financial/Resource Management

• Participates in PHP budget process annually.
• Recommends budgets in alignment with short and long-term plans.
• Manages resources to ensure priorities are accomplished.
• Recommends changes which result in more effective use of resources and cost containment.
• Ensures approval for budget variances as required.
• Approves resource allocations within budget, including people, finances, and timelines; make decisions on exceptions.

Relationship Management

• Leads effective communication system for work group(s), ensuring a collaborative culture.
• Builds and ensures effective relationships across internal teams and external organizations for current or future integration.
• Develops and maintains partnerships with community-based social service organizations to assure CareOregon can leverage and improve access to upstream non-medical health-enhancing services for its members.
• Partners with internal leaders and managers in identifying improvement plans and processes.
• Represents CareOregon in external meetings and functions, providing productive leadership presence and effectiveness

Employee Supervision

• Directs team(s) and establishes team direction and goals in alignment with the organizational mission, vision, and values.
• Identifies work and staffing models; recruits, hires, and oversees a team to meet work needs, using an equity, diversity, and inclusion lens.
• Identifies department priorities; ensures employees have information and resources to meet job expectations.
• Leads the development, communication, and oversight of team and individual goals; ensures goals, expectations, and standards are clearly understood by staff.
• Manages, coaches, motivates, and guides employees; promotes employee development.
• Incorporates guidance from CareOregon equity tools into people leadership, planning, operations, evaluation, budgeting, resource allocation, and decision making.
• Ensures team adheres to department and organizational standards, policies, and procedures.
• Evaluates employee performance and provides regular feedback to support success; recognizes strong performance and addresses performance gaps and accountability (corrective action).
• Performs supervisory tasks in collaboration with Human Resources as needed.

Experience and/or Education

Required

• Minimum 8 years’ experience in social health and/or community health settings
• Minimum 4 years’ experience in a supervisory position

Preferred

• 5 or more years’ experience in the housing or food services sector
• Experience working with or for a health plan or Coordinated Care Organization (CCO)
• Advanced training and/or coursework in a field related to the position

Knowledge, Skills and Abilities Required

Knowledge

• Expert knowledge of the intersection between diverse medical and psychosocial problems and situations of poverty, societal discrimination, and intergenerational trauma
• Knowledge of managed care and publicly-financed health care stewardship principles
• Understanding of health literacy, shared decision making, and both the delivery and impact of health related social needs like housing and food
