Job Summary
The Community Health Manager is responsible for the management and continuous improvement of “social determinant” and community health program initiatives within the Jackson Care Connect CCO (JCC). This position is responsible for overseeing outreach to Jackson Care Connect members and the governance of the Community Advisory Council and serves as a liaison to the JCC Board of Directors to provide program updates. This position is responsible for establishing and maintaining vital partnerships as a representative of the CCO across different social service and public health. This position also manages the development and implementation of community wellness programs and health equity strategies, including identifying opportunities to partner with and/or invest in innovative, community-oriented programs that improve upstream health and social determinants for vulnerable or at-risk populations in our community. Central to this role is ensuring the sustainability of programming through program design and alignment with organizational objectives.
