The Bilingual Community Health Worker will play a pivotal role in promoting public health and well-being in Adams County. The Bilingual Community Health Worker will co-learn with community members and adapt health promotion programs to honor cultures and experiences. This is an exciting new position working with the Extension Rural Health Connect project with the Division of Extension Health & Well-Being Institute.
This role requires a passion for community well-being, strong communication skills, and the ability to adapt to the unique needs of different communities. As a Community Health Worker, you will be at the forefront of promoting positive health outcomes and fostering a healthier, more resilient community.
Key responsibilities include:
– Participate in training and development to support position responsibilities.
– Deliver evidence-based health promotion programs addressing various health issues like nutrition, physical activity, mental health, substance misuse prevention, and health literacy.
– Serve as a resource for community members seeking health information.
– Develop and maintain relationships with community members and partners.
– Collect data and report on impacts.

The Extension Rural Health Connect Project is supported by the Rural Partnership Institute at the University of Wisconsin-Madison with funding from the United States Department of Agriculture. The Extension Rural Health Connect Project seeks to establish an Extension-based community health worker network to improve whole family health in rural Wisconsin communities.

Instructs and/or assists individuals and families engaged in various outreach programs. Delivers research-based educational programs to support and empower people where they live and work. – Spanish speaking, writing and comprehension skills; knowledge and understanding of Hispanic/Latino culture. Language skills will be evaluated.
– Ability to deliver health education to individuals and in group settings.
– Knowledgeable about community resources for health and well-being.
– Strong interpersonal skills and demonstrated ability to build and maintain professional work environments.
– Ability to work independently and as part of a team.
– Ability to bend, stoop, kneel, lift 30 pounds, move and work on your feet for periods of time
– Ability to effectively work with people from different cultural backgrounds, including those associated with race, ethnicity, national origin, religion, socioeconomic status, age, gender, disability, sexual orientation, and other aspects of human diversity.
Preferred:
– Volunteer or work experience related to public health, community health, health education or a related field.
– Experience managing multiple tasks, timelines, and schedules while maintaining high quality work.
– Ability to work cooperatively with other agencies, community-based services, or organizations.
– Community Health Worker Certification.

Additional Information:
Length of position: The initial period of employment will be until December 2026, with the possibility to become renewable or be extended contingent upon available funding.
Position location: This position offers partial remote work based in Wisconsin. It may require some work to be performed in-person, onsite, at a designated campus work location, or at a remote, offsite, non-campus work location. Periodic travel in Wisconsin may be required.
Applicants are required to have: A valid driver license, Reliable source of independent transportation for periodic in-state work travel, and/or complies with requirements for UW-Madison vehicle use authorization. Staff are expected to make individual arrangements for transportation adequate to meet position responsibilities and essential job functions.
This position may be required to work occasional nights and weekends to meet the program needs and priorities of the communities served.
