Position Summary:
The UNC Formerly Incarcerated Transition (FIT) Program is a partnership between UNC Family Medicine, the North Carolina Department of Adult Correction (which administers the state prisons), Federally Qualified Health Centers, County Departments of Health, and local reentry councils. Upon release from the state prison system, individuals are frequently without work, without health insurance, and without a medical home. The FIT Program aims to connect all formerly incarcerated individuals who have a chronic disease with a primary care provider. The program hinges on Community Health Workers (CHW) making contact with individuals who are incarcerated with chronic disease prior to release, developing rapport, and helping put together a comprehensive reentry plan. Since its inception, FIT has taken on additional projects that align with the program’s mission to improve health outcomes and decrease recidivism for formerly incarcerated people.
In 2021, FIT received a grant from the North Carolina Department of Health and Human Services to provide technical assistance and support to organizations across the state who were awarded funds to implement pre- and post-arrest diversion programs, comprehensive re-entry planning and navigation support programs, medication-assisted treatment in jails, overdose prevention, and other linkage-to-care initiatives with justice-involved people. Our technical assistance (TA) team is comprised of FIT (team lead), the North Carolina Harm Reduction Coalition (NCHRC), and the Duke Opioid Collaboratory. Based upon the success of the grant, the TA team was contracted by NCDHHS to continue this work and create a TA Center to support agencies across the state who serve justice-involved individuals.
The TA Project Coordinator will assist with all aspects of the TA Center during its initial startup and all future phases. This position will coordinate project development and implementation and serve as the TA team’s main point of contact for all correspondence, scheduling, and meeting management. The TA Project Coordinator will assist with project evaluation and tracking and monitoring project deliverables. The position will work closely but independently with key stakeholders, including but not limited to, UNC faculty and staff, NCHRC, the Duke Opioid Collaboratory, the North Carolina Department of Adult Corrections, Federally Qualified Health Centers, County Departments of Health, and local community organizations and reentry councils. This position will function as a critical member of a highly integrated and interdisciplinary team and work closely with the respective units.
Minimum Education and Experience Requirements:
Relevant post-Baccalaureate degree required (or foreign degree equivalent); for candidates demonstrating comparable independent research productivity or comparable background in independent academic or instructional activities, will accept a relevant Bachelor’s degree (or foreign degree equivalent) and 3 or more years of relevant experience in substitution. This experience may be related to the direction/oversight of programs that focus the academic and research resources of a higher education institution towards community or regional issues.
