The ALS Association Hiring for Care Services Coordinator at Bozeman, MT  Full Time

SUMMARY: The Care Services Coordinator provides education, information and resources, as well as coordinating and delivering services to individuals with ALS and their families.

DUTIES AND RESPONSIBILITIES: Core duties and responsibilities include the following. Other duties may be assigned.

• Resource and Support
  • Provide consultation to people with ALS and their families concerning equipment needs, information, resources, referrals, and ways to maintain or improve quality of life
  • Work in collaboration with medical professionals (OT, PT, SLP, RT and others) concerning equipment needs, accessibility, mobility, and resource options for people with ALS and their families
  • Acquire, organize, and maintain updated information on ALS, ALS specific research, patient care techniques, and equipment
  • Accurately document all engagements with people living with ALS, their caregivers, and other contacts in the Chapter database (Salesforce) in a timely manner
• Chapter Program Services

• Advise and refer people with ALS and their families to chapter services/programs
• Attend ALS certified clinics as needed and help to expand access to clinics in MT
• Field questions regarding equipment, helping to identify correct pieces of equipment for people with ALS, and facilitate pick up or delivery of the equipment
• Engage with SLP, OT/PT, RT and other medical professionals to ensure proper training is conducted for all people living with ALS that receive equipment
• Manage and monitor loan locker inventory as needed
• Plan and facilitate ALS support group meetings (if not the group facilitator, work closely with support group facilitators for pre-meeting planning and follow up afterward)
• Provide in home or virtual visits for people with ALS and their families as needed

• Education

• Educate people with ALS and their families regarding equipment and technologies for people living with ALS
• Engage medical professionals and community partners on pertinent topics and educational opportunities related to ALS
• Represent the Chapter at professional education conferences and participate in continuing education seminars and workshops, as necessary.
• Collaborate with Chapter Development staff in regularly submitting/writing articles for the monthly newsletter
• Provide in-service education programs to local home health agencies, hospitals, skilled care facilities, and other community partners

• General

• Represent the Chapter by attending, and assisting at, various chapter events which involve persons with ALS and their families.
• Make recommendations to the Care Services Director, and/or the Care Services committee regarding the development of new programs and services
• Assist the Care Services Director as needed, including care services related tasks that may extend outside of MT
• Keep current with state licensure and National board registrations (if applicable)

SUPERVISORY RESPONSIBILITIES:

• This job has no supervisory responsibilities.

QUALIFICATIONS:

• 3+ years of experience in social and human services, social work, healthcare, medical or other related field, ideally including experience with ALS, MNDs, or other progressive and terminal illnesses
• Reside in Montana, preferably Helena, Billings or Great Falls areas
• Knowledge of home accessibility, resources and environmental controls, and assistive technologies and/or willingness to learn
• Ability to assess both physical and psychosocial needs of ALS patients and families, based on knowledge of ALS, the disease, and its ramifications.
• Possess knowledge of community resources.
• Possess excellent organizational skills.
• Possess strong communication skills.
• Possess basic computer skills (Microsoft Word, Excel, Outlook, PowerPoint).
• Experience with SharePoint and/or Salesforce a plus.
• Flexibility and availability to work some evenings and weekends as needed for special programs.
• Maintains personal auto insurance.
• Proof of COVID vaccination required

The ALS Association’s pay range for this position is $46,191 – $57,597 annually.

The ALS Association has compensation ranges for various work locations throughout the United States, allowing us to compensate employees competitively and consistently in diverse geographic markets. The above-listed listed range shows prospective compensation for this role; the appropriate range will be established by what is eventually determined to be the candidate’s primary work location. Various factors, including the complexity and significance of the role, job duties/requirements, relevant experience and abilities, and primary work location, determine individual compensation. Compensation ranges are evaluated and typically adjusted on a regular basis. Offers are made within the compensation range that is in effect at the time of the job offer.

The Association may, at its discretion, offer merit-based increases tied to individual performance. The benefits listed may differ depending on employment status with the Association.
