Job Summary
The Manager of Rehab & Subacute Case Management provides overall management of the daily operations for the Inpatient Rehab unit including Case Management, Care Transitions Coordinator, Rehab Therapies and Nursing. Accountable for staffing plan and associated budgets. All employees are expected to be knowledgeable and compliant with Prisma Health’s values of compassion, integrity and teamwork.
* Coordinates the delivery of patient care for assigned unit/services and ensures the quality of care provided. 20%
* Demonstrates a broad knowledge of clinical practice using innovative critical thinking skills. Demonstrates knowledge of inpatient rehab Case Management functions including but not limited to working knowledge of medical necessity criteria, patient management, case management documentation systems, and all other systems used in the daily case management and admissions activities. Responsible to work directly with all disciplines to ensure development and coordination of staff education, team building initiatives and case management strategies. 15%
* Works collaboratively to identify deviations from standard practice and to implement or develop interventions to support service recovery. Works with staff to mentor, advise and support effective change in response to identified barriers. 10%
* Responsible for leading initiatives that will identify potential concerns that may cause dissatisfiers for patients, customers, and staff and developing programs that will enhance satisfaction for these populations.10%
* Acts in a managerial role related to Human Resources activities including staff management, policy and procedure development, and performance evaluations of staff. Responsible for interviewing and hiring of staff as well as performance coaching for existing staff members. 20%
*Develops, analyzes, monitors and revises staffing patterns for the unit and ensures that sufficient and properly prepared personnel are available to meet patient needs. 10%
* Responsible for financial management activities for the department to include the development and management of applicable budget processes. 5%
* Responsible for the management of data collection initiatives that will garner qualifiable data illustrating the impact the department is having on clinical case management. Responsible for planning statistical audits and performing root cause analyses that will add integrity to data collected and managed by the department. 5%
* Responsible for the management and reporting of all regulatory compliance efforts with various applicable regulatory agencies. 5%
* Develop and implement policies and procedures that guide and support the provision of the services. 5%
* Prepares, implements and evaluates unit goals and objectives. Develops, implements, monitors and evaluates personnel performance objectives on an annual basis. 5%
Job has direct and/or indirect supervision of employees that may include final budget authority, hire/termination/disciplinary authority, performance appraisal responsibility. Job will be considered a member of the management staff with direct reports.
Bachelor’s Degree – Nursing
3 years -Case management, Medical surgical nursing, home health, or population health.
RN – REGISTERED NURSE – NLRN
Knowledge of PPS, MDS, and regulatory agencies – preferred.
Master’s Degree, 1 year leadership experience – preferred.
Certified Case Manager or Accredited Case Manager- preferred.
Knowledge of Medical Necessity Criteria (Interqual, MCQ) – preferred.
