FedPoint, previously doing business as Long Term Care Partners, LLC, was founded in 2002 and is a versatile administrator of large-scale insurance and benefits programs. We are hiring a Care Coordinator to join our team! This is an exciting role working in a 400-person company with big aspirations, and a wonderful culture. We are offering a 100% remote opportunity for this position.

The Care Coordinator is responsible for benefit eligibility and/or recertification decisions, identification and coordination of care, education of claim program and processes, providing information, and offering referrals. This position interacts directly with the insured or their legal representative, providers, and health care professionals to provide accurate, timely, and responsive replies and resolution.

The Care Coordination Department serves as licensed healthcare practitioners and support personnel in providing client advocacy and general information about long term care services. This department is responsible for answering Federal Long Term Care Insurance Program enrollees’ inquiries and assessing and approving their need for long term care services based on the program criteria.

Required:

• Registered nurse with 5+ years’ experience in a variety of medical settings, preferably in long term care or home health settings.
• Familiar with standard concepts, practices, and procedures within the field.
• Reviews the claim and POC and updates accounts as necessary.
• Basic computer skills with MS Office Suite.
• Completion of the AHIP Long Term Care courses within 3 years of employment (in house program). Supports and participates in the mandatory training on an annual or more frequent basis, as required.
• Required to uphold the principles of compliance and fraud management as outlined by corporate and departmental policies.
• Utilizes sound professional judgment. Demonstrates empathy and responsiveness. Excellent telephone interpersonal and communication skills.
• Excellent assessment skills and professional judgment.
• Ability to work independently with minimal supervision.
• Flexible with ability to handle multiple tasks simultaneously.
• Experience working as an independent practitioner.

Preferred:

• Experience in Case Management preferred.
• Good telephone skills – previous telephonic experience a plus.
