Commonwealth of Massachusetts: Executive Office of Health and Human Services Hiring for Statewide Head Injury Program (SHIP) Coordinator at Worcester, MA  Full Time

The Massachusetts Rehabilitation Commission (MRC) is seeking a dynamic individual to fill the role of Statewide Head Injury Program (SHIP) Coordinator. The selected candidate is a professional eager to make a difference in the lives of individuals with brain injury and their families. The SHIP Coordinator is responsible for working with individuals with brain injuries to allow them to remain as independent as possible in the community. Support is provided to individuals and their families, and may include assessing needs for services, arranging for services and coordinating these, and working with other involved professionals. In addition, they act as a liaison to programs, agencies, and organizations by developing assigned areas of expertise such as substance use and children’s services by reviewing topical information, attending advisory boards and task forces that meet to discuss issues as they relate to individuals with disabilities in order to develop expertise and provide technical assistance. They monitor program services including review of consumer progress reporting, regular contact with consumer’s families, vendors and program staff, site visits and participation in case conference. They coordinate referrals, admission transfers to and discharge from appropriate programs. They provide outreach, information and resources to consumer’s families, public and private agencies, legislators, and other officials. The SHIP Coordinator is also responsible for fiscal monitoring service utilization.

One position is located in Central Massachusetts and will include work with homeless individuals. Cases are occasionally assigned out of region.

Duties and Responsibilities (these duties are a general summary and not all-inclusive):

• Engage assigned consumers, their guardians and families as appropriate, in a person-centered planning process with careful consideration and accommodations for communication or accessibility needs.
• With assigned consumers, guardians and families as appropriate, in cooperation with assigned regional and residential providers, develop a plan for services that includes the goals of the consumer and services to support attainment of these goals.
• Coordinate referrals, admissions, transfers to and discharges from appropriate programs for assigned consumers; complete all required documentation, including client plans and service arrangements within the required or requested timeframe.
• Monitor program services as assigned; provide support, education and recommend training to provider agency staff; provide informal functional supervision; support development of community resources that will meet individual needs.
• Monitor and lead consumers’ annual service planning meetings, ensuring that all applicable standards and regulations are observed.
• Initiate, oversee and respond to all incident reports within required timeframes; identify trends to ensure for health and safety through contact with the consumer and/or provider for service needs or changes, mandatory reporting requirements, and coordination of consultation as needed. Review and record provider incident reports in a timely and accurate fashion; communicate all incidents involving possible neglect or abuse to DPPC, ensuring supervisors are notified of these; complete DPPC and Agency death reporting forms as required within the required time-frame.
• Review program invoices and reports to ensure that services are provided as contracted; discuss any concerns with supervisor immediately.
• On a quarterly basis or as needed and as directed by supervisor, communicate with the individual, family or guardian and update the service plan as necessary. This includes the need for and authorization of services and maintaining timely and accurate case notes.
• Prepare budget recommendations for consumer services and monitor on-going fiscal expenditures in collaboration with supervisor.
• Maintain all client files, including electronic files, in an orderly and accurate fashion. Complete updates in database as required. Utilize tools and systems developed by the agency, program and team to carry out responsibilities. As needed, engage in training to improve technological skills.

Required Qualifications:

• Ability to travel statewide is required.

• Excellent communication skills in oral and written form, solid follow-through, flexibility.

• Ability to clarify and to resolve problems and conflicts, and the ability to self-organize and manage priorities.

• Ability to work comfortably with remote technology (computer, VPN, remote calling) is required.

• Familiarity with the needs of individuals with disabilities living in the community is a must.

Preferred Qualifications:

• Knowledge of brain injuries is a plus.

• Bachelor level training in social work or psychology is desired; MA level training is a plus.

Minimum Entrance Requirements:

Applicants must have at least (A) three years of full time, or equivalent part-time, professional, administrative or managerial experience in business administration, business management or public administration the major duties of which involved program management, program administration, program coordination, program planning and/or program analysis, or (B) any equivalent combination of the required experience and the substitutions below.
