The Nurse Practitioner works independently with collaborative supervision using intuitive skills when problem solving in complex clinical situations.
He/She demonstrates a highly skilled analytical ability in novel circumstances. He/She is credentialed to function in an expanded nursing role. He/she
is responsible for assessment and management of patients, including diagnostic and therapeutic interventions and the implementation and evaluation
of an appropriate plan of care. All activities are performed within the guidelines of protocols agreed upon by the Medical Director and under medical supervision. He/She participates in clinical research and/or the utilization of its results. He/She is involved in multidisciplinary intramural and extramural consultation for the delivery of patient care. All nursing practice is based on the legal scope of practice, national and specialty nursing standards, CNMC Policies and Procedures, and in accordance with all applicable laws and regulations. The Professional Model of Care requires registered professional nurses to be responsible and accountable for their own practice. CNMC supports the development of RN relationships within the community; specifically those relationships associated with the health and well being of the community at large.

Minimum Work Experience
2 years Experience as a Pediatric Nurse Practitioner. (Required)

Functional Accountabilities
Strategic Planning

• Role will work with teams that are focused on redesigning care delivery models, utilizing Advanced Practice Providers while raising the bar on patient quality and safety; accountable for reporting assessments and making strategy recommendations to Program Manager.

• Role will work with teams to enhance Advanced Provider infrastructure, informatics, equipment, and processes related to technology as they relate to Advanced Practice Providers. Accountable for reporting assessments and making strategy recommendations to Program Manager.

• Integrate Quality Improvement methodology into Advanced Practice Provider culture through tools, training, and hardwire principles into our organization to make processes more efficient; accountable for reporting assessment and making strategy recommendations to Program Manager.

• Contribute to the sustainability and advancement of nursing excellence.

Clinical

• Assess individual patient clinical status by obtaining complete medical examination; developmental assessment; ordering appropriate laboratory tests, and x-rays as indicated; to diagnose, monitor and promote clinical status of each patient, utilizing evidenced based practice and medical knowledge.

• Gather and maintain patient medical records including lab, MRI, EEG results, and abnormal lab findings; interpret results and plan interventions accordingly.

• Develop appropriate plan of care.

• Collaborate with other health care team members, clinics, and community agencies; address patients care needs.

• Perform minor procedures when indicated (e.g. ear irrigation, suture removal).

• Independently prescribe legend drugs according to guidelines.

• Responsible for onboarding and orientation of all new APRN and PA providers in SBHC Department

• Relay important and timely updates to staff in regard to operations of the APP practice meetings and APP practice at CNH.

• Provide Monthly Staff Schedules.

• Ensure Monthly Timesheets are accurate for review by the SBHC Manager.

• Liaison to department(s)/division(s) leadership and Montgomery County School Based Health Administrators.

• Identify learning needs of staff. Develop appropriate in-services/projects to address identified needs.

• Participate in development of NP role and practice within organization.
