Shift Hours:
Part-Time: 20 hours/week, Monday-Friday
Job Description:
Responsible for addressing the psychosocial needs of patients by utilizing consultative services, psychosocial assessment, and comprehensive intervention plans. Coordinates the treatment planning process for patients in the pre-, intra-, and post- care settings, as feasible; coordinates follow up services post discharge; collaborates with other members of the treatment team to assure the continuum of care.
Qualifications:
Wisconsin Social Work license required.
Skills in assessing, planning, and managing patient care in an acute care hospital or other health care facility recommended; background in care management; knowledge of available community resources to assure the continuum of care is beneficial.
