Vibra Hospital of Western Massachusetts – Central Campus, located in Rochdale, MA is a 47 bed Critical Care Hospital providing acute care services to patients recovering from serious illnesses or injuries. Often our patients require additional critical care services for medically complex conditions such as trauma, infectious diseases, wound healing, cardiovascular disease, stroke, amputation and ventilator weaning. As a smaller, specialized hospital, we offer an opportunity to work in a fast-paced and friendly work environment where you really get to know your patients and their families.
Responsibilities
Provides psychosocial assessments and a full range of social work services to patients and their families on an intermediate care locked unit. Provides counseling, and discharge planning. Acts as a liaison between the patient/family/hospital and community agencies/resources. Demonstrates expertise in social work, working as a member of an inter-disciplinary team comprised of a physician, nurses, and case management and devices treatment plans to help patient work towards discharge. Applies specialized knowledge of human development and behavior and social, economic, and cultural systems in directly assisting patients and families to improve or restore their capacity for social functioning. This includes counseling and use of psychosocial interventions and social psychotherapy. Occasionally plans educational offerings and provides in-services as requested.
Required Skills:
Master’s Degree in Social Services required
License eligible, supervision provided.
Additional Qualifications/Skills:
Licensed Clinical Social Worker (LCSW) or LMHC preferred.
Two (2) years of experience as a Social Worker in a healthcare setting preferred.
Acute care experience preferred.
Current BLS certification from a Vibra-approved vendor preferred.
Ability to project a professional image.
Knowledge of regulatory standards and compliance requirements.
Strong organizational, prioritizing and analytical skills.
Ability to make independent decisions when circumstances warrant.
Working knowledge of computer and software applications used in job functions.
Freedom from illegal use of and effects of use of drugs and alcohol in the workplace
