Summary
This 1.0 FTE position; 70% administrative role focused the management of our primary care and anticoagulation teams, 30% effort dedicated to clinical services. This position reports directly to the Director, Clinical Ambulatory Pharmacy Services, Pharmacy Innovations and Partnerships.

Responsibilities*
The Clinical Pharmacists Manager, Primary Care and Anticoagulation Clinical services assists with maintaining the clinical business for Pharmacy Innovations and Partnerships in accordance with departmental standards, internal and external reporting requirements, accreditation standards, and pharmacy laws and regulations. The Pharmacy Manager ultimately oversees the primary care and anticoagulation clinical pharmacy teams to ensure the teams are providing optimal patient care and adhering to health-system policies. The Manager provides oversight of pharmacy practice programs, medication use systems, and quality initiatives to detect, mitigate, and prevent medication adverse events and work to ensure medication use is aligned with evidence-based best practices and regulatory standards. The incumbent also works with interdisciplinary partners to deliver medication therapy management within patient care services and to ensure pharmaceutical care programs are appropriately integrated.
