Required Education:

Bachelor’s degree in social/behavioral service field

Required Related Experience:

2+ years previous experience working with adult protective services and/or vulnerable adult populations

Physical Requirements:

• Lifting, pushing, or pulling of objects up to 25lbs.
• Sitting for long periods of time.
• Repetitive motion typical of desk work (typing, sitting, bending, twisting, prolonged screen exposure).

Shift:

Monday – Friday 8am – 5pm
(30 – 40 hours per week)

Job Summary:

This position is responsible for assessing individual self-neglect in adults as part of the College of Social Work Adaptive Response Initiative, in partnership with the Department for Community Based Services (DCBS) Adult Protection Services Branch (APS). This position will serve as part of the DCBS APS team to provide oversight and evaluation of multidimensional, multidisciplinary assessments of functional ability, physical/cognitive/mental health, and socioenvironmental factors to determine required level and frequency of services. This position is responsible for the management and evaluation of individual plans of care, in collaborationwith community partners, designed to promote independence, autonomy, and dignity.

Skills / Knowledge / Abilities:

The successful candidate will be knowledgeable about Adult Protection Services.

• Demonstrates an ability to work independently in engaging the vulnerable adult toassess and identify strengths and needs within established timeframes to includeconducting face-to-face interviews, home visits, and gathering collateralinformation.
• Demonstrates an ability to work independently in coordinating services to vulnerable adults to ensure that appropriate services and referrals are completed timely and thoroughly to promote safety, well-being, and permanency.
• Demonstrates an ability to work independently in maintaining professional communication and relationships with internal and external community partners.
