Summary of Job Duties:Come join our dynamic therapy team focused on providing innovative one on one patient care. This position is for an Outpatient Physical Therapist (PT) on the main campus. The PT will be responsible for the assessment of patients to positively impact functional deficits in collaboration with referring physicians and clinicians. Treatment will focus on best practice care planning and home programming. Qualified applicants will have a desire to work with an innovative team focusing on skill advancement, continuing education, research opportunities, and community engagement.
The physical therapist provides an individualized evaluation, during which the client, family/caregiver, and physical therapist determine the patient’s goals, customize interventions to optimize the person’s ability to perform functional activities, and continuously assess progress towards goals to ensure goals are being met and/or to modify the intervention plan based on the patient’s needs and skills. Functional outcomes data is completed through collection of best practice data via patient questionnaires, performance measures, and documented treatment interventions.
Skills include but are not limited to the following: evaluation and establishing plans of care, therapy reassessments to adjust plans of care when appropriate, functional performance reassessment for discharge planning, screenings to determine appropriateness of therapy, daily treatments which may consist of: gait training, transfer training, functional mobility, balance training, endurance training, strength training, therapeutic exercise, pool therapy, functional performance testing, orthotic measurement, orthotic fitting and adjustment, and patient and family education. Appropriate patient documentation per department, HIPAA and JCAHO standards where applicable.Qualifications:
