POSITION SUMMARY:
The Manager of Social Care is responsible for the overall development, application, and implementation of Mercy Care’s Social Care programs, to include wrap around services for patients, case management, complex patient programs (high utilizers of the ER, substance abuse support, etc.) in order to achieve lasting health improvement through comprehensive, patient centered care that effectively addresses the social influencers of health. This position will have responsibility on care model development, program analysis, evaluations, development and adherence to quality metrics and partnership development. Responsible for training and development to support the skills and knowledge of the social care team. This position hires, supervises, and evaluates competent staff to carry out a wide variety of functions including direct patient care and case management. The Manager of Social Care will assist with the development and implementation of a multidisciplinary team structure and model of care to support non-clinical case management services across the organization. With support of senior leadership, this position will develop and implement a patient advisory council and professional development plan for Social Care, as well as foster relationships with external organizations to meet patient needs. The Manager will foster partnership across the metro Atlanta community to better support Mercy Care patients and to ensure that Mercy Care is available to serve those supported by our partner organizations.
EDUCATION REQUIREMENTS: Masters level social work, counseling, psychology, sociology, etc. preferred, with accompanying educational degrees as required for licensure under the Georgia practice acts.
EXPERIENCE REQUIREMENTS: At least seven years’ experience working in a human services environment, including at least five years in program development and administrative/supervisory role(s) serving disadvantaged or special needs population(s). Experience working in a multi-disciplinary team environment, as well as in program development grant writing, grant management and evaluation is required as well as proven experience in development of quality metrics. The ability and legal right to drive in Georgia, including an active, current Georgia Driver’s License may be required.
JOB KNOWLEDGE: Must possess strong organizational, management and leadership skills, including supervision of staff, employee assessment, problem solving, hiring, training, project documentation, monitoring, reporting, and evaluating and disciplining of employees. Must have knowledge of and ability to develop and maintain relationships with community resources. Must have good time management skills and able to maintain appropriate professional and personal boundaries and be a positive, role model for patients and colleagues. Computer literate and working knowledge of applicable software programs, (i.e. Microsoft word, Excel, etc.). Financial analysis to include ability to analyze financial statements and identify inefficiencies within a budget. Racial and cultural sensitivity is essential.
SUPERVISORY RESPONSIBILITY FOR: Non-clinical case management roles throughout the organization, including but not limited to Community Health Workers, Outreach workers, Resource Specialists, and Direct Service Case Managers. May also have joint supervision of case management for complex populations.
