Care Management Coordinator
Behavioral Health Services – Access Clinic South (1635 National Ave.)
Annual Salary: $48,547.20 to $73,278.40
Job Summary: Under the direction of the Care Coordinator Supervisor, the Care Management Coordinator will provide crisis care coordination to individuals who are identified as requiring a period of psychiatric stabilization for a period of one to six months. Individuals in need of further stabilization, and/or individuals awaiting placement into a long-term level of care are identified by Crisis Services. Services are provided in the community, i.e., at the person’s resident, doctor’s clinic, etc.

Knowledge, Skills and Abilities: Knowledge of social work principles and practices; knowledge of case management/assessment techniques, knowledge of mandatory reporter requirements; knowledge of community resources and services available; skill in communication in verbal and written form; skill in interpersonal relationships; skill in organizing office work and cases; ability to communicate case needs to staff; ability to work in stressful situations; ability to collect and analyze data and summarize in a useful/appropriate format; ability to comfortably use word processing, databases, spreadsheets and email systems; ability to work as a member of a mental health team providing crisis services; able to maintain confidentiality; ability to lift, carry, push, and pull up to 15 lbs. as needed.
