With over 700 employees, Behavioral Health Services (BHS) at Denver Health is a large and rapidly expanding department that provides psychiatric care for patients in community, emergency, inpatient, justice, medical, mobile, and outpatient settings. As a non-profit committed to meeting community needs and promoting health equity, Denver Health offers psychiatrists a practice full of meaning and purpose. All physicians at Denver Health have an academic appointment at the University of Colorado School of Medicine. Job Summary
Under general professional supervision, performs behavioral health case coordination services including planning, organizing, and treatment referral/disposition. Delivers services in accordance with the approved plan of treatment and plan of care. Assists behavioral healthcare providers in understanding significant social and emotional factors related to the patient’s behavioral health issues.
The THRIVE Peer Recovery and Crisis Team works to improve treatment engagement among high-risk patient populations with behavioral health conditions, with a focus on those who are experiencing homelessness and/or exiting corrections. The Team provides linkages to care, warm hand-offs during acute care episodes and/or correctional care releases, accepts referrals from outreach teams, offers crisis intervention and assessment, and follows patients for eight weeks to support treatment entry.
