Summary:
The Wellness & Prevention Community Health Worker (WP-CHW) is primarily responsible for helping Members and their families navigate and access preventative care/wellness services, community resources and adopt healthy behaviors via multimodal attempts, including telephonic calls, text campaigns, auto dialer and Member portal messages. As a priority, activities will promote, maintain and improve the health of Members. Functions in collaborative practice with CCHP staff, healthcare professionals, community partners and the Member population. Skillfully and consistently performs duties per established policies and procedures.

Education and Experience:
High school diploma or equivalent required.Associate or bachelor’s degree in healthcare, human services/development, or closely related field preferred.
Minimum of three (3) successful years of experience in community health, social services, community advocacy, community outreach, health plan Member Services and/or Engagement, or a combination of relevant experience and education.
Understanding of the community in the Service Delivery Area through shared experiences and/or a strong desire to help people in vulnerable communities.
Community Health Worker (CHW) certification required.
Must be bilingual with fluency in both English and Spanish with excellent oral and written communication skills. Pass a Spanish proficiency examination prior to hire.
Computer and data entry experience required, as well as proficiency in Microsoft Office products (to include Word, Excel, and Office).
Ability to work well without direct supervision, independently and productively.
Effective telephone skills.
Ability to follow-up and follow-through with strong attention to detail.
Good communication skills, such as listening well and using language appropriately.
Licensure, Registration, and/or Certification:
TX Community Health Worker certification required.
