POSITION SUMMARY: The Intake Coordinator, in conjunction with the other members of the admission staff, is responsible for activities and actions related to the admission, communication, and staffing for the start of care of referred patients. Needs to be a driven and goal-oriented individual that can organize, coordinate, and manage all referrals and admissions.
REPORTS TO: Intake Manager, Director of Operations, or Executive Director
SUPERVISES: None
QUALIFICATIONS:
Education: High School Diploma or equivalent
Experience: At least one year of health care experience is preferred.
Core Competencies: Knowledge of state and federal regulations and requirements including Medicare, Medicaid, and private insurance guidelines. Good communication and marketing skills. Ability to respond to the needs of physicians, discharge planners, patients and family members.
FUNCTIONS & RESPONSIBLITIES:
1. Works directly with nursing and office staff to coordinate start of care for members.
2. Receive and respond to referrals through use of phone, fax, and insurance portals.
3. Verifies and obtains all appropriate information for admission including, but not limited to, eligibility/benefits.
4. Maintain accurate and up-to-date referral log and patient referral folders.
5. Coordinates appropriate durable medical equipment to successfully deliver products in a timely manner.
6. Collection of the order, the authorization and the history and physical.
7. Monitor and update status of referred members.
8. Obtain and update demographics for members with incorrect or missing contact information.
9. Provide daily reports of received referrals and outcomes of scheduled admissions.
10. Complies with policies and procedures, laws and regulations and standards of practice.
11. All other tasks and duties deemed necessary and appropriate.
