Job Description:

Centerstone is looking for a compassionate Health Home Clinician in Alton, Illinois.

Health Home Clinician:

• Provides in-office and community case management services (e.g., phone calls, home visits, housing and financial counseling; referrals/linkages)
• Provides individual and group therapeutic interventions that facilitate personal growth, skill building, identification and use of natural supports, and use of community resources
• Responsible for assisting with assessment and personal growth planning, linking clients to appropriate services, advocating on their behalf, providing support, monitoring effectiveness of services and providing crisis services as needed.
• Participates in a variety of cross-functional teams and performance improvement initiatives.

Qualifications/Requirements:

• Five (5) years of paid experience in the mental health or related field.
• Bachelor’s degree in a human service related field.
• Must have own reliable transportation, liability insurance, and valid driver’s license
