OPPORTUNITY
We have a unique Social Worker opportunity in our new PACE program. This key role contributes a psychosocial perspective to the interdisciplinary evaluation, assessment, plan of care and ongoing services of older adults.

Responsibilities:
Assesses the prospective participant and the enrolled participant in the comprehensive initial, semi-annual and change of condition assessments and formulation of the plan of care.
Refers prospective and enrolled participants with community social service resources to meet care gaps needs. Maintains current information related to community resources, federal and state guidelines.
Collaborates with the interdisciplinary team to maintain effective, efficient, and appropriate care to optimize the health status and quality of life for the participants.
Applies social system skills to implement interventions to meet the needs of the participant, caregiver, and family. Facilitates participant and family adjustments to lifestyle changes.
Provides ongoing counseling and crisis intervention and addresses mental health needs as they arise.
Assists in providing education for the New Horizons Living associates, volunteers, participants, caregivers, and families.
Maintains up-to-date and appropriate documentation in the participants electronic medical record.
Participates in the interdisciplinary team meetings, communicating participant changes, collaborating on care planning decisions and coordination of 24-hour care delivery.
Participates in in-service training and staff meetings as required.
Performs other duties as assigned.
Qualifications:
Minimum
Masters degree in Social Work.
Medical clearance for communicable diseases and up-to-date immunizations, before having direct participant contact.Current drivers license, auto insurance, and dependable transportation.
Desired
(2) years experience working as a MSW in a health care or social services field.
1) year working with the elderly population.
Current American Heart Association Basic Life Support (BLS) with Automated External Defibrillator (AED) certificate or obtain within two weeks of hire, before having direct participant contact.Estimated Salary: $20 to $28 per hour based on qualifications.
