POSITION SUMMARY:
Under the supervision of the Ambulatory Clinical Pharmacy Manager and according to department policies, procedures, and guidelines will be responsible for providing the highest level of direct patient and customer care to patients undergoing treatment at the medical center. Clinical Specialist will also be working under the clinical supervision of medical director. Clinical specialist ensures optimum patient related outcomes for clinic patients by preventing or resolving drug-related issues and optimizing the drug therapy of patients. Provides drug information to patients, physicians, nurses, and other healthcare professionals. Integrally involved in education of pharmacists, residents and students. Additionally, the Clinical Specialist coordinates clinical projects, assists with formulary coordination and policy revisions, ensures billing compliance and revenue capture and helps to lead other projects as needed.
Serves as a resource and drug expert for all general internal medicine related medications and strives to increase knowledge through research and writing activities.
The Ambulatory clinical pharmacist specialist will be responsible for providing clinical pharmacotherapy services to patients under the care of our outpatient pulmonary clinic. The specialist will work closely within multi-disciplinary pulmonology and pharmacy teams in the evaluation, development, and implementation of various clinical protocols, chronic disease state management under collaborative practice agreement, provide extensive patient counseling, and provide leadership/mentorship to clinical pharmacists. Specifically, this role will look to provide pharmacotherapy interventions to patients at high risk for admission and readmission to the hospital related to COPD.
Job essentials:
Evaluation and management of chronic conditions through direct patient care working under a collaborative practice agreement within the outpatient pulm clinic.
Teaching and precepting APPE 6th year PharmD students, PGY-1 and PGY-2 practice residents
Collaboration with interdisciplinary healthcare professionals to improve and implement transitions of care (TCM) programs.
Involvement with various quality improvement projects, pharmacy department initiatives and provider education
Actively participate in ambulatory formulary review process and various institutional committees
