Requires every weekend.
This hospital uses Epic for its EMR.
You’ll work with adult patients of all ages, pediatric patients, and psych patients for case management.
A vaccine declination is accepted for those with religious or medical exemptions.
In this nurse case manager position, you’ll execute the hospital’s discharge planning or utilization review, or a dual role for both.
You’ll implement a long-term care plan for patients in collaboration with the hospital’s medical team.

Requirements Section Valid CA state license (Your recruiter can help advise you on how to obtain the proper state license and required credentials for this position and in most cases, we can reimburse you for the fees).
Facility will interview applicants without a license.
