DESCRIPTION
Under
general supervision, the Pharmacist II is responsible for provision and documentation of medications and medication-related services to
patients according to standards established by the Department of Pharmacy. The pharmacist II will serve as preceptor to pharmacy residents
and students as assigned and will participate in Medical Center and Departmental committees and promote institution-wide medication use
policy.
The Pharmacist II will be responsible for providing direct patient care to patients in various populations at UCSD Health
through the transitions of care model. The pharmacist will work in collaboration with providers, perform medication reconciliation, counsel
patients upon discharge, conduct post discharge follow ups, and other duties as assigned. The pharmacist may work under Collaborative
Practice Agreements to provide optimized patient care, patient education/counseling, optimize therapeutic outcomes, and address adherence
barriers. The target patient population may include (but is not limited) internal medicine patients, cardiology patients, patients with
HIV.

variable shifts to include weekends, holidays and overtime as required.
Demonstrate knowledge of pharmacy state and federal
regulations; interpersonal and customer service skills; supervisory skills to include workload management; ability to operate computer
systems; demonstrate teaching skills and writing skills. Have excellent communication skills with the ability to communicate clearly both
verbally and in writing and be able to articulate complex ideas for all levels of audiences. Work with the physicians and the pharmacy staff
to provide justification for obtaining authorization for medications.
