Mass General Brigham Home Care serves patients from rehabilitation after illness or injury, to chronic disease management. This position will work 30 hours total on Friday (8hrs) , Saturday (11hrs) and Sunday (11hrs).

Ensures weekend operational functions and activities are completed and provides guidance to administrative staff and clinicians on the weekend team. Ensure appropriate utilization of high-quality home care services, maximize staffing mix, and participate in case conferences, triage incoming customer contacts, support management activities, perform supervisory visits. Provide management oversight of LPNs, per-diem Clinicians and Schedulers. Precept and educate new weekend staff and act as a mentor and resource. May provide direct patient care as needed.
Key Responsibilities
* Staff on-boarding and ongoing development will be identified, and training implemented as needed and as directed by the manager. Assist with precepting and education needs
* Staff performance is monitored and evaluated, and routine performance feedback is provided to staff. Staff performance is regularly communicated to manager and guidance is sought when necessary. Participation in annual performance review process for staff by providing feedback to manager.
* Performance appraisals for LPNs, per-diem clinicians and Schedulers accurately reflect employee performance and are delivered within established time frames.
* Team and individual accomplishments are recognized, appreciated, and communicated appropriately.
* Employee survey results are analyzed and interventions are implemented to improve scores
* Oversight of assignment of staff is coordinated in order to match the needs of the patients and staff skills, as well as meeting geographic efficiency.
* Clinicians are supported and guided to effectively handle complex patient cases..
* Patient care needs are communicated to assigned clinicians providing information necessary for patient care.
* Clinical staff training needs are identified and training is coordinated with staff education and specialty nurses, when appropriate agency guidelines and policies are communicated to staff.
* Clinical documentation is reviewed for completeness, accuracy and consistency to ensure compliance and adherence with established criteria.
* Clinician field visits are conducted at least annually for each staff members and more frequently as directed by the manager for teaching and problem resolution.
* Patient referrals may be evaluated and accepted according to staff availability and established policies and procedures.
* Case conferences are conducted regularly with staff when requested
* Computerized systems are utilized to enhance delivery of care to achieve agency goals and outcomes.
* Case mix and utilization are reviewed and tracked to optimize financial outcomes. Trends are identified and recommendations are made for improvements.
* Clinical and operational issues are identified and communicated to Manager timely.
* Customer issues related to team members are communicated and/or resolved with appropriate action plan and in a timely manner while referred to the manager as appropriate
* Provide coverage for Weekend Clinical Manager as Clinical Coordinators as needed
Education/Degree Requirements
* Bachelor of Science in Nursing
* 3- 5 years progressively responsible nursing experience with at least 2 years in community health
* Prior supervisory or management experience required.
