Job Summary: Continuity of Care Navigators perform moderately complex coordination work, provide preliminary assessment of a client’s presenting psychiatric and/or co-occurring disorder(s), develop a plan to provide linkage to services sought or needed by the client, and to coordinate appropriate services to facilitate each client’s success in achieving and maintaining the maximum possible level of independent functioning. This position is community-based and provides short term services to both with both adolescent and adult clients admitted to/discharged from the local network private psychiatric hospitals in Travis County. The services provided will focus on linkage to resources and aftercare, decreasing re hospitalization and ensuring that contractual obligations pertaining to continuity of care are being met.
This position serves individuals diagnosed with severe mental illnesses who may also have co-occurring substance use disorders and/or chronic health conditions utilizing a recovery based and trauma-informed perspective. The services that Continuity of Care Navigators provide require frequent coordination with external and internal providers is necessary to support individuals in achieving independent functioning. Continuity of Care Navigators provide coordination and discharge planning to assure that the individual is discharged to the least restrictive environment. Services may include coordination to assist the client in obtaining or maintaining employment, housing, medical care, dental care, food, clothing, utilities, financial assistance, transportation, legal assistance, or any other basic human need.
Although staff members in this position are not typically LPHAs, staff members in this position are working under the supervision of a LPHA who provides ongoing evaluation and oversight of all assessments, crisis intervention, caseloads and client coordination. This position may have additional duties associated with providing bilingual communication services and requires the completion of a language competency test. Interventions and services are typically provided in the community and require the use of a personal automobile and cell phone. This position requires work availability for holidays and weekends on a rotational basis.

Regular, predictable attendance is considered to be an essential requirement for this position. Other duties may be assigned. May require a flexible work schedule.

Salary: $22.35 – $23.47 an hour based on education.
